I tried to install Burnout on my HDD using OPLs (v1.2.0-Beta-DB-2158) NBD server + HDLdump (0.9.0), which was successful many times before. Unfortunately my connection dropped this time and the game was not fully transferred. When I fired up OPL and refreshed the HDD list, I had two blank entries at the top of the list and Burnout somewhere lower. I've deleted Burnout and then the third blank entry appeared at the top. I've looked at the HDDmanager in wLaunch but found nothing suspicious. Then a silly idea came up, to rename this blank entry. After changing its name to "1", the whole list of games disappeared. Then I checked HDDmanager again, and it only said that the drive is connected but not formatted :/ I assume that some partition headers were damaged.
I tried to open the drive in wxHexEditor, and the beginning is in the attached file below. It's "APA" with suspicious number "1" which maybe coincidentally is the string I tried to change the blank entry to. Reading Berions APA documentation, i thought that first few partitions should be sysconf system etc. one after another, and on mine drive i have "__empty" and then the game "PP.SLUS-20721.HDL.R_Racing_Evolu" then "P.APA __mbr" and blank data for a long time.
Then I used a spare SSD to format it in HDDmanager and check the stock layout, which turned out to be default partitions one after another. At this point I'm confused about this partition layout and whatever it's salvageable without reformatting the whole thing.
I have Linux with wxHexEditor (or maybe you have something better to work with hard drives) and pfsshell. Below I provided beginnings of Hex dump from faulty and template drives
Edit: It turns out that I am not able to paste links, so I can send it in private message later.
