You cannot merge disc game, his DLC and patch into one disc game. Best you can do without risking game breaking at some point is just preparing PS3_EXTRA with like name suggest, extra content. :)

Or, making your own package with patches and DLCs. Or your own archive (*.pkg is cancer format).

Eventually, use digitally distributed games. Those can be merged fine (with few exceptions where DLCs gets their own GameID dir).
 
Because:
- they need to be installed, which means twice space needed for use them
- decrypting and decompressing is long process
- sometimes there are many patches for one game and many duplicating data
- CellOS not supporting anything but FAT32 for external storage, which means ~4GiB file limit, which means you will not install pkg above this limit without splitting pkg into parts

Very true, but also there are some advantages using games in PKG format:
- They don't need a backup manager like webMAN MOD or multiMAN/IRISMAN/ManaGunZ
- They can be launched without CFW, qCFW or HEN enabled.
- They can be organized by albums or removed easily from XMB.
- No blackscreen issues.

There is a workaround to let install large PKG files (>4GB) from NTFS or exFAT external drives:
1- Copy the large PKG file to /PKG in the external drive formatted in NTFS or exFAT
2- Install webMAN MOD + prepISO (if they are not already installed).
3- Run prepISO to scan the content from the external drive
4- After scan complete, install the PKG file listed in the folder "Blu-ray and DVD" of XMB > Game > webMAN Games

Some games can run from external FAT32 or ps3netsrv if the are extracted to /GAMEI folder. However the compatibility depends of many factors: it may not work correctly if the game writes to the game folder if is launched from ps3netsrv or if the game has internal files larger than 4GB if it's stored in FAT32. Some PKG games only run from the internal HDD.
 
To opposite for PKG I have in mind unpacking them on PC, then transferring, then rebuilding database. Not disc images. ^^ But yeah, some of those points also not makes user independent (due to use needs something for transfer data into internal disk like FTP server or file manager) if he not have or cannot dump ERK.
