PS3 problem to insttal 4.91 after format dd

I have a problem with the PS3, Well I installed the game GTA Andreas, and after trying to launch the game, the ps3 console crashes with black screen. I restarted the device, asking me to reinstall the system again, but it stops at 8%. Then I formatted the hard drive and inserted a flash drive with a 4.91 update into it, and when starting it asks me to press Start + Select. But this doesn't respond gives me anything about the system. The controller works fine but start + select doesn't give me anything. Is there a solution to this problem? THANKS. my dear friends

Check first S.M.A.R.T of HDD.

Reinstalling fw must be done via recovery menu. Well, if You zeroed HDD You have no choice anyway.
