PS3 Making custom pkg patch for custom title id game

I'd like to make custom patches for my custom title id game. I've tried to make it with ps3tools, but game doesn't load any of changed files install via custom pkg patch. Is it even possible what I'm trying to do? If not, could custom pkg patch be made for already existing title id?
