PS3 Mount any ISO makes my PS3 completely frozen or black screen

New PS3 jailbreak noob here, bought a pre-own one and following the video from Modded Warfare and MrMario to mod my ps3 which model number is CECHH00 using CFW, but after installed, i notice that my PS3 went freeze when i mount an ISO by webMAN MOD, first i wait it for almost 1 hour then i realize its a abnormal process which takes so long, and the power LED just keep green blinking forever, i have no choice but force shutdown by holding down the power button for a while until i heard 1 beep and release then it shutdown.
Power on the PS3, it asked me to repair the file system, i say yes, then go on, no issue, then went to the XMB main menu, i choose that ISO again in webMAN MOD, and it just works, the game runs fine without any issue and i even play it for an half-hour (the game is ICO from PS3 not the one from PS2), then i power off my PS3, and power it on again, then the frozen issue cames again, so weird.

I searched the whole web and psx-place found no one had this issue, what should i do next to fix this? here is the detail about my jailbreak steps:

1. The PS3 CECHH00 runs OFW 4.86 CEX when i first power it on, I'm using the ps3toolset.com to jailbreak it, i setup my web browser settings for the jailbreak process (set blank page, clean every cache), then enter the page, wait it loads until pops up 4 donation qrcode and check the log if there is any red text that i need to redo it again, then i backup the OFW flash first before patching the flash memory and checked that using PyPS3Checker and it's all green no problems, then i patched the flash and reboot my PS3.
2. Download the Evilnat 4.91 Beta 10 PEX CFW from psx-place's post, choose PEX, MD5 is good, then copy it to my usb drive, plug it into PS3 usb port on the right, flashing it from XMB system update and it reboots, shows the evilnat logo boot animation.
3. Installed the multiman and webMAN MOD downloaded from brewlogy, the multiman version is「multiMAN (BASE) v04.85.01 10/10/19」and the webMAN MODversion is「webMAN MOD [4.91] v1.47.46h 08/21/24」and there is no other homebrew to be installed
4. First i ran the multiman then i quit to the XMB, then i ran the webman mod installer, pressing L1 for full version, then webman mod shows up in XMB
5. Using WinSCP to transfer the game via webMAN mod built-in FTP feature, refresh in webman mod not work so i ran multiman then i quit to XMB and the game shows up in webman mod.
6. Launch it or mount it via webman mod, then my monitor display frozen or stuck after about 1 second or half-second, leaving a message stuck at the top right corner says /dev/hdd00/xxxx.iso something like that and ISO is loaded, PS3 LED blinking green forever.
7. Force shutdown PS3 by holding the power button, it plays 1 beep after i pressing it for about 2-3 second then i release the button and it shutdown.
8. Power on PS3, it asked me unsafe shutdown for the last time and need to check the file system for data, press yes, wait it finish, then get into the XMB menu.
9. Run the same ISO via webman mod, but this time we ripped off that issue, the disk icon shows up in game section, and when run it, everything is good.
10. Power off PS3 then power it on again the frozen issue came back, at first i absolutely have no clues about how i successfully run it on the last time, till i manually trigger the repair file system by going to the「Custom firmware tool」utility on XMB's Network section.
11. So everytime I power on my PS3, I need to trigger the repair file system manually and wait it finish until i could play my game in ISO format.

There is some other useful information about what i tried to do to fix it and for everyone diagnosing this issue:
- Tried to downgrade to Evilnat 4.88 PEX, but no luck, and i leave the QA flag enabled.
- Tried to clean the dust and replace the thermal paste on both IHS surface and Die surface, yes i did the delidding, and i achieved it with success for the first time XD, but my PS3 gots very clean when i opened it up for the first time, the fan is almost brand new, and there is no dust inside, the original thermal paste is dry out so i literally just need repaste it with new one. before this, PS3 runs at 85 celsius for both CELL and RSX with syscon controlled 20% fan speed on XMB main menu, after this it runs 55 celsius for both with the same settings. the seller said that he rarely use it and need to clean up his room space so he sell it to me.
- Tried to replace the original 40GB HDD with new one from my old laptop Western Digital 250GB, which is still work perfectly and the SMART of new HDD is good, but no luck.
- Tried to do a PS3 Full reset in safe mode and lost all my homebrews also my games and install multiman only, then transfer the game via FTP, mount it via multiman, then multiman gaves me a black screen with a constant green LED light. need to hold down the power button for a long time till i heard 3 beep then it shutdown immediately.
- Tried to mount ISO via IRISMAN but no luck.
- Tried to mount ISO via ManaGunZ but no luck.
- Tried to convert ISO into JB folder format and mount it via any hb above but no luck.
- Tried to rollback to OFW 4.91 and redo the whole jailbreak process, patching flash, and install Evilnat 4.91 Beta 10 PEX, and install CFW again via XMB system update, and AGAIN via safe mode, yes, i flash it even three times, but all of those were out of luck.
- Tried to mount ISO from external usb drive but no luck for both ISO format and JB folder format. (ISO itself is fine, tested after repair file system, it runs great)
- Tried to reset to default settings and rebuild database and PS3 full reset on Evilnat 4.91 Beta 10 PEX but no luck, only repair file system works for this boot.
- The only thing that works is to install the game with PKG format, the game runs with no issue, no black screen or frozen, you don't need to repair file system before run it.
- I also notice that PS3 would get frozen when i insert a disc to the Bluray drive if i don't repair file system first, but the disc could be eject when PS3 frozen, the disc is a DVD disc, after repair file system the disc could be read without issue.

I'm running out of ideas, what should i do?
 
Everything it's very weird, and you didn't make a clean test (too many factors introduced at once, like using multiMAN and WebMAN MOD at the same time).

Better start anew, with a clean installation and no homebrews (you can format your HDD and then use the CFW PUP from Evilnat). Then, you can go first by installing WebMAN MOD and mount your ISO.

If everything is good and you don't get any black screens, maybe you had some issues with your HDD partition, your OS installation or homebrews messing with some system files.
If something goes wrong, then you may have hardware issues (could be the HDD, the power supply, something with the motherboard, can't tell for sure).

That's weird because there is no Evilnat 4.88 PEX. I think you mixed your PUPs while trying to downgrade.

If you want the most stable CFW, you better use REBUG REX 4.84.

Thanks for the reply, i will try my best to control variarites on it and give it a more clear explaination. by the way, i also notice that when i rollback to OFW, i inserted a dvd movie disc it would cause the PS3 frozen or stuck just like in Evilnat CFW, but again a force shutdown and do a repair file system then it will work, the disc is fine, reading without issue. there is definitely got an issue outside of PS3 system since OFW and CFW both have the same symptom when user is trying play with disc function.
Should i rollback my original PS3 flash to my OFW 4.86 backup too, and jailbreak it again? i would start a new installnation with clean test as you say

As your model uses NOR memories, there is no need to reflash it. Just format your HDD on the computer and the FW PUP will install as usual (either be CFW or OFW).

At this point, better stay with CFW REBUG 4.84, as is the most stable.

From the symptoms you are describing, maybe the HDD, the BD drive or the SATA port are causing them (can't be 100% sure).

One test you can try, after setting up a clean install, is rip your DVD to an ISO image, mount it with WebMAN MOD and see if the console crashes. That way you can discard if it's a software issue or something else.

So i tried REBUG 4.84 but issue is still there. and with many clean installation tries in last 2 days like multiman only and webman only but no matter what i do, the PS3 was frozen dead after mount ISO. i supsect that was casued by BD drive. but with NOBD CFW from Evilnat it still freeze my ps3 (I flashed it as usual but not disconnect it in physical).
And now, i'm tired of this, start thinking it was a hardware issue, sure i can play PKG, but for ISO i just need to repair file system and wait a little bit longer on booting.
Anyway, thank you for your awesome advice, i'll continue to work on it later.
 
Good news: I might have fixed that strange behavior.
And for bad news: I didn't find the root cause, and actually I don't remember what I just did or which step was the most important.

Here is the detail:
The base CFW is Evilnat 4.91.2 Beta 10 PEX running on CECHH00, flashed twice via XMB system update in order to reset ROS1 and ROS2.
First you did a file system repair in custom firmware tools menu, reboot and mount a PS3 game iso using Webman MOD, and quit the game, go to the webman settings, tick the auto load last game on boot (sorry my native language is not english i dont know if it is right or not), then reboot your PS3, you should get an ISO mounted message on right cornet but it will failed, then untick the auto load then shutdown the PS3.
REMOVE THE BD DRIVE inside of PS3 and turn it on, then try to launch any applications will gives you a black screen, then you have to force shutdown but you might not get the unsafe shutdown message and prompt to repair file system on next boot.
Now flashing the Evilnat 4.91.2 Beta 8 noBD PEX twice via XMB system update (if you flashing it directly via Safe mode it might stuck at scanning the USB so it's better via XMB), then launch those apps again it will work, but not for the ISO, it will failed with black screen.
On next boot, when prompting to repair file system press yes and if it is not, do it manually in custom firmware tools menu then power off the PS3, reassemble the BD drive and when booted hold down the eject button for at least 30 seconds, you might hear some noise from BD drive while holding, then insert a DVD disc or any other disc that could be read by PS3 you should get nothing on XMB, eject the disc and mount an ISO and prepare the black screen then force shutdown.
Here is the most interesting part, by insert a disc when PS3 is in power standby state (red led light on), your PS3 will power on instantly but the BD drive will refuse to accept the disc untill boot process is completed. you might seen nothing on XMB on this boot when you insert a disc.
Now flashing the Evilnat 4.91.2 Beta 10 PEX twice via XMB system update and performing a file system repair by custom firmware tools menu your ISO is now working again also for the BD drive function.
Reboot your PS3, this time, your PS3 should black screen when mount an ISO or read disc, but for me, IT'S WORKING NOW, no freeze and no black screen. and i don't need to repair file system first before i mount ISO also for the BD drive reading disc.
