The app that @Hexxy Chan is talking about is the firmware updater, WHICH CLEARLY SAYS IN IT'S DESCRIPTION UNDERNEATH THE DOWNLOAD OPTION that it is to be used with Overflow 4.81 cex2dex or Overflow 4.81 dex2cex versions of the said cfw.
Just for clarification, the app that Hexxy Chan mentioned does not edit the flash, All this app does is update the VSH and install some small files designed to update the cfw and spoof the ps3 for psn access.
As we all know installing an incorrect VSH file will cause a soft brick as it will not be signed correctly, however it is not possible to cause a hard brick, if there genuinely is some issue causing the ps3 to not boot into safe mode it must be a coincidental hardware issue.
From what Hexxy Chan says there seems to be some other issue, for example not using a wired ps3 controller for safe mode.
Strange how the ps3 booted into safe mode but now does not, my guess is graphics issue/video reset needed.

In the description of that video it has a written explanation of how to convert from cex to dex or vice verca.
If anyone is interested,
Install overflow 4.81 cex2dex v1.
Install Overflow boxed tools from the install packages->hard drive option.
Open boxed tools, go to cex and dex options.
Select dump eid root key.
When ps3 beeps click convert to dex.
When ps3 restarts go to whats new in psn column and install cfw updater.
When ps3 restarts it is all done ready to go online.
When converting from dex to cex, it is the same process but click convert to cex (the option will have changed).
