PS3 Strange issue while using HEN

I am encountering a strange issue while using HEN. My current system configuration is 4.93 HFW + HEN 3.50, though I experienced the exact same problem previously on 4.92 HFW with HEN versions 3.4.1 and 3.4.2. I am using a Super Slim model, and I currently have a copy of *Final Fantasy XIII* inserted in the disc drive. Upon booting up the console, the system sometimes recognizes the disc and sometimes does not; it seems to be completely random. If the disc is recognized, everything functions normally after activating HEN. However, if the disc is *not* recognized, activating HEN leads to problems: there is a high probability that all homebrew applications will freeze. Furthermore, attempting to load PSX ISO images also results in errors.
Update: If the disc is not recognized, opening and closing the disc lid does not resolve the issue.
 
Maybe the disc is scratched?
Maybe the laser has a problem with reading the disc?!
Do the other discs work without a problem?
 
the drive itself shouldn't cause this problem. on older ps3, it would be the daughter-board dying. apps and games would black-screen freeze cause sony makes them do it if the drive is bad or not detected. maybe there is a intermittent power issue or other bad connection.
