To me, this sounds eerily similar to the Avira bug described in the Avira support forum.
My guess is that exceptions are working fine except when they are not in place before a first false positive comes up AND the false positive is not dealt with by setting an exception directly from the "Virus Found" pop up & refusing to add the file to Quarantine...
When that happens, Avira recognises the exact file version, most likely using its checksum, and keeps getting rid of it automatically despite the exception settings.
Something along those lines...
There must be a way to reinitialize the way Avira deals with the PKG_ContentID.exe file to work around this issue though...
